INSURANCE

If you bring your insurance card(s) with you, the front office staff will be happy to fill out insurance forms for you to file. We will be happy to help you receive maximum benefits. Remember, the agreement with the insurance company for medical care is a contract between you and the insurance company, not between the insurance company and Dr. Carr. It is a method of reimbursing you, the patient, for fees paid to the doctor and not a substitute for full payment. Some companies pay a fixed allowance for certain procedures and others pay a percentage of the cahrges. It will be your responsibility to pay any deductible amount or other balance not paid by the insurance company.

Even if insurance has been filed, you will automatically receive a monthly statement from us. This will let you know the status of your account.

Since our creditors require prompt payment from us, we cannot carry accounts, insurance or private pay longer than 60 days.

MEDICARE

We accept assignment on all services. Insurance forms are filled out for all Medicare Supplemental Policies.
